Your root canal specialist: Dr Ahmad Mahmood
Root canal treatment at Dental Plus is led by Dr Ahmad Mahmood, the clinic's lead consultant and a root canal specialist in Rawalpindi. He holds a BDS, an MDS in Endodontics and Restorative Dentistry, and C-Implantology training. An MDS is a postgraduate specialist degree taken after the BDS. Endodontics is the specialty that deals with the pulp and root canals inside a tooth.
That training matters most in the harder cases, such as a complex molar root canal with three or more canals, narrow or curved canals, or a tooth where an earlier root canal has failed. Missed and difficult canals are among the reasons the American Association of Endodontists gives for root canals that do not heal.
Patients notice it. In their Google reviews, patients call him "a skilled endodontist". They say he "keeps himself updated about the latest technologies" and "gives his patients the due time required".
He works in a clinic that has been treating patients since 1984, alongside two consultant orthodontists and a consultant prosthodontist. So the crown that often follows a root canal is done in the same place.
Signs you may need a root canal
- Severe pain when you chew or bite
- A pimple-like bump on the gum
- A chipped or cracked tooth
- Sensitivity to hot or cold that lingers after the food or drink is gone
- Swollen or tender gums
- Deep decay or darkening of the gum
A tooth infection usually starts with deep decay, gum disease, a cracked or broken tooth, or an abscess. An examination, usually with a dental X-ray, shows whether the pulp, the nerve inside the tooth, is infected.
How our dentists manage it
How we manage your root canal treatment
Confirm the diagnosis
An examination and X-ray show whether the pulp is infected and whether the tooth can be saved. We show you what we see before suggesting treatment.
Explain the plan and cost
You know how many visits to expect, whether the tooth will need a crown afterwards, and the cost, before we begin.
Treat it comfortably
The tooth is fully numbed, and the canals are cleaned, shaped and sealed over two or more visits, with a temporary filling in between.
Follow up and protect the tooth
We check how the tooth has settled at your follow-up visit and fit a crown if it needs one, so the tooth lasts.
What happens during treatment
- 01The tooth is numbed with local anaesthetic. You're awake but shouldn't feel pain.
- 02A small opening is made in the top of the tooth.
- 03The infected pulp is removed, and any abscess is drained.
- 04The canals inside the root are cleaned and shaped.
- 05The canals are filled and the tooth is sealed. A temporary filling may be placed between visits.
You may need a crown afterwards to protect the tooth, especially if the infection was severe. Dr Ahmad Mahmood, our endodontist, will tell you whether you need one.
Recovery
Numbness usually wears off in 2 to 4 hours, and the American Association of Endodontists advises not eating until it has completely gone. Some soreness or pain after a root canal is normal, and swelling around the tooth should improve within two weeks. Most people go back to work or school straight afterwards.
- Take paracetamol or ibuprofen if you need pain relief
- Avoid knocking or bumping the tooth
- Keep brushing and cleaning between your teeth as normal
- Come to every follow-up appointment
How long does a root-filled tooth last?
According to the American Association of Endodontists, most teeth that have had root canal treatment can last a lifetime with proper care. Keeping your natural tooth means normal chewing, biting force and feeling, a natural look, and no extra strain on the teeth next to it.
Root canal or extraction: which is better?
When a tooth is badly infected, the choice is usually between saving it with a root canal or taking it out. The American Association of Endodontists says saving your natural tooth is the better option whenever possible. Here is how the two compare:
| Root canal treatment | Extraction | |
|---|---|---|
| Your natural tooth | Kept | Lost |
| Chewing and feel | Normal biting force and sensation | Needs a replacement tooth to restore |
| Neighbouring teeth | Protected from extra wear and strain | Can drift into the gap |
| What comes next | Usually a filling or a crown | An implant, bridge or denture |
| Overall cost | Usually lower | Usually higher once the gap is filled |
If a tooth truly can't be saved, we'll tell you, and talk you through extraction and dental implants.
Re-treating a failed root canal
A root-filled tooth can become infected again, sometimes months or years later. This is called a failed root canal. Common reasons are narrow or curved canals that were not treated the first time, or canals that were missed. A crown fitted too late can also be the cause, as can a crown or filling that cracked or came loose and let bacteria back in. New decay or a fracture can also cause it.
Re-treatment means reopening the tooth, removing the old root filling, checking for missed canals, then cleaning, refilling and sealing them. The tooth then gets its permanent restoration, usually a crown. If re-treatment isn't possible, the alternatives are endodontic surgery, or extraction and an implant.
Who treats you
Root Canal Treatment at Dental Plus
Root canal treatment here is led by an MDS endodontist. Narrow, curved or missed canals are among the reasons a root canal fails. Treating root canals is what an endodontist does every day.
Toothache at night? We're open 24 hours a day, 7 days a week. At an emergency visit we can examine the tooth, ease the pain and start the root canal, then finish it at follow-up appointments. See what to do in a dental emergency.
Patients come to us from across Rawalpindi and Islamabad when a root canal done elsewhere has failed, and we can often re-treat it. Crowns after root canal treatment are part of the plan, so you won't need to go elsewhere for them.

Common questions about root canal treatment
How much does root canal treatment cost in Islamabad?
Prices published online for Islamabad start at around PKR 8,000. Marham, a Pakistani doctor-listing site, gives PKR 8,000 to 15,000 per session in Islamabad. The total depends mostly on the tooth. A November 2025 Pakistan price guide lists about PKR 7,000 to 18,000 for a front tooth and 12,000 to 25,000 for a premolar. For a molar, it lists 20,000 to 40,000 or more. A crown is charged on top. At Dental Plus you get the exact fee for your tooth after the examination and X-ray, before any treatment starts.
What is the normal cost of a root canal?
There is no single normal price, because the work depends on the tooth. A front tooth usually has one canal, while a molar has three or more. That is why the same guide prices front teeth from about PKR 7,000 and molars from about PKR 20,000. It also lists prices by technique. A rotary root canal costs about PKR 12,000 to 25,000, and one done under a microscope PKR 20,000 to 30,000. If the tooth needs a crown, the guide puts the root canal and crown together at PKR 30,000 to 70,000 or more. Taking the tooth out is not always cheaper. The American Association of Endodontists notes that an extraction often means paying for an implant or bridge later, which can cost more.
Why 3 visits for root canal?
Not every root canal needs three visits. The NHS says to expect two or more, each lasting one to two hours. Extra visits usually have a clinical reason. When the nerve has died or there is an abscess, the canals are often filled with a calcium hydroxide dressing for about a week. This kills the bacteria that remain. One study found this extra disinfection raised healing rates by about 10% at one year. A flare-up between visits can also be dealt with before the canals are sealed. A typical three-visit plan is: clean and dress the canals, then fill and seal them, then fit the crown. Research reviews find single-visit and multiple-visit treatment about equally successful, when each is used for the right tooth. So we plan the number of visits for your case and tell you before we start.
Is RCT of teeth painful?
Usually not. The tooth is fully numbed with local anaesthetic. According to the American Association of Endodontists, most patients say a root canal feels like having a routine filling. The severe pain people link with root canals is normally caused by the infection itself, and treatment is what relieves it. Afterwards, mild soreness or tenderness is normal for a few days, especially if the tooth was painful before. Paracetamol or ibuprofen help, and the NHS says swelling and soreness should improve within two weeks. Our patients describe their root canals here as "painless" and "pain free".
What happens if I don't get it treated?
The infection will not heal on its own, and without treatment the tooth may have to be taken out.
Can a failed root canal be treated again?
Often, yes. Re-treatment removes the old root filling, cleans the canals again and reseals them. An examination and X-ray show whether the tooth can be saved.
How long after a root canal can I eat?
Wait until the numbness has completely gone, usually 2 to 4 hours, so you don't bite your cheek or tongue. After that, chew on the other side until the tooth has its permanent filling or crown.
How long does a root canal take?
The NHS says to expect two or more visits of 1 to 2 hours each. Simpler front teeth can sometimes be finished sooner, while molars with several canals take longer. We tell you how many visits your tooth needs before we start.
Can I go back to work after a root canal?
Usually, yes. The American Association of Endodontists says most people return to work or school straight after. Your mouth may stay numb for a few hours.
Can I get a root canal at night or on the weekend?
Yes. Dental Plus is open 24/7, so if an infected tooth is keeping you awake, call or WhatsApp 0306 5574644 and come in. We can ease the pain and start treatment the same visit. See our 24/7 emergency dentist page.
Will I need a crown after a root canal?
Often. A crown protects the tooth, especially after a severe infection. We tell you before treatment starts whether yours is likely to need one.
Is it better to just take the tooth out?
Usually not. The American Association of Endodontists says saving your natural tooth is preferred. It keeps normal chewing and bite, preserves the bone around it and stops the neighbouring teeth from shifting. An extraction then needs an implant, bridge or denture to fill the gap, which can cost more and take longer.
